Be specific and detailed in your response.           Part II: Walk Through a Change to Improve Quality Using the IHI Model for Improvement What quality measure needs to improve in your workplace or clinical site? Pick one that allows an evidence-based, nurse-driven change. Use this measure to work through the MFI (model for improvement) discussed in the IHI lessons.  ·              Set an aim, provide the full statement. Use the worksheet provided in the lesson if needed.     AIM Statement:   ·              Stakeholders: This would be anyone who could feel the impact of or contribute to this project, typically will cover a broad variety of people or groups of people (including financial stakeholders).  List the stakeholders and how this QI problem and/or change process impacts them. (Hint: Do not forget the client/patient) StakeholderImpact                          ·              Team: Who will need to be involved in this process for approval, data collection, implementation, etc.  (Hint: It needs to be multidisciplinary.)  Who will lead the team? How often will the team meet? This group will be unique to the setting and the change being implemented.       ·              Changes (evidence-based)- Search the literature using the UARK library Quick Search regarding how to improve the quality measure in question. Filter for the last 5 years and include the exact setting for specific interventions. (Example to type in the search: Improvement of Falls rate in skilled nursing facilities) What evidence turns up in this search?  What intervention(s) does the evidence support?
