Introduction
The purpose of this research project is to discuss the effectiveness of the Teach-Back method in the delivery of discharge education on readmission of heart failure patient within 28 days upon discharge from hospital settings.Identified Issue
Heart failure is estimated to affect 5 million people in the United States according to statistics from the Centers for Disease Control and Prevention (Ryan, et al 2014).
