Assignment #4 Evidence-Based Self-Change Report
I. Briefly describe the practice problem and evidence-based practice question. (5 pts)
II. Describe the way you searched for the best evidence (internet search engines, websites, abstracts, etc.) (5 pts)
III. Briefly describe each chosen intervention (Plan A and Plan B). (10 pts)

VI. Formulate a single-case design to evaluate your own effectiveness in implementing Plan A. (10 pts)
VII. Describe and justify what you will measure, source of data (Self-report? Behavioral observation? etc.), any measurement instruments used (i.e., reliability? validity?), who will measure, and when each measurement will be taken. (15 pts)

Is there a visual pattern that depicts a series of coincidences in which the frequency, level or trend of the target problem changes only after the intervention is introduced or withdrawn?

1.     In column A, enter the measurement intervals by which you will take the measures (ie, days, weeks, sessions.
2.     In Column B enter the values that correspond to the measurement interval
3.     Highlight the data in column B, select Insert, go to Charts and click on the Insert Line, the Line Icon.
4.     Under 2D select the fourth icon, Line with Markers.
5.     Next, in the Insert tab, select shapes. In shapes, select the line.
6.     Once you have the line, draw the line on the graph to show the baseline and the intervention phase.
7.     Now you have visual representation.
